Abdominal aortic aneurysm - improving outcomes for patients

Closing the Gap through Clinical Communities
The Abdominal Aortic Aneurysm Quality Improvement Programme was initiated after it was found that the mortality rate for abdominal aortic aneurysm (AAA) surgery in the UK was 7.5% compared to other European countries at 3.5%. The project aimed to reduce mortality rates in the UK to 3.5% by 2013.

Airedale NHS Foundation Trust: Conserving a scarce resource and improving outcomes

Shine 2010
The team at Airdale NHS Trust were concerned with the high rates of blood transfusions for joint replacement operations for certain subgroups of patients, and the lengthier stays in hospital this causes. To mitigate this they are introducing optimisation of haemoglobin concentrations prior to surgery, and are putting into practice a transfusion limitation strategy.

Yorkshire and Humber Programme for Shared Haemodialysis Care

Closing the Gap through Changing Relationships
Every year around the UK 20,000 patients with kidney disease receive dialysis – cleansing the blood by passing it through a machine. This usually takes three-to-four hours at a time, three times a week, either until the patient receives a transplant or for the rest of their life. This project empowers dialysis patients to regain some independence, through training patients and staff to work in partnership to help individuals manage their own care.
