The site has been delivering the Co-creating Health programme with a focus on people with musculoskeletal pain.
Eight sites participated in phase one of the Co-creating Health programme, seven of which are continuing to work with us in phase two. Each site spans primary and secondary care and involves local teams of people with long term conditions, clinicians and managers.

Also focusing on COPD, the site has been working with respiratory inpatients and outpatients at Addenbrooke’s hospital and with defined GP practices within the locality.
Focusing on self management support for people with diabetes, the site is delivering Co-creating Health across primary and secondary care.
Initially focused on self management support for people with COPD in secondary care, the team have since involved GP practices on the island of Arran and a number of practices in Ayrshire.
The team delivered phase one of the Co-creating Health programme with a focus on people with musculoskeletal pain.
The site has been working to promote self management support for people with depression. They have liaised with user and voluntary groups to access hard-to-reach clients, including prisoners and single parent families.
The teams have focused predominantly on improving self management support for people with depression who are being seen in primary care.
Focusing on self management support for people with diabetes, the site is delivering Co-creating Health across primary and secondary care.
