As the population ages, it is important for people to stay healthier for longer so that the proportion of life lived in poor health reduces, thus improving quality of life and reducing demand for services.

Many people see their GP for all their health and wellbeing concerns. However, GPs are often unable to help with underlying issues such as social isolation, money or housing concerns. Time and capacity to navigate local health, social and voluntary services are limited. These wider factors directly impact on health and wellbeing, leaving patients vulnerable to physical and mental health issues, and inequalities.

Live Well Greenwich operates at three levels: universally available online Greenwich Community Directory; universally available Live Well Line (telephone support service); and targeted intensive Live Well Coach (LWC), supported by community volunteers.

This project developed a network of LWCs trained in using motivational approaches and social prescribing, addressing wider social and economic needs. A training pathway for future LWCs was further developed with the Royal Society for Public Health.

Early evaluation is promising, with 75% of those contacted to meet an LWC booking an appointment. Available outcomes data show that 97% of patients were at least in the early stages of change, and 62% were at the ‘action’ stage.

Qualitative data from clients and GP practices was very positive. In addition, significant support and referrals were provided by the Live Well Line, demonstrating the benefit of a multi-tiered service.

Further resources have been committed from partners to expand the service.
